Staff Training

The surgery will be closed for Staff Training on Thursday 24th September from 1.00pm-6.00pm.   The surgery will re-open on Friday 25th September at 8.00am.  If you require advice wile the surgery is closed please call NHS111 or 999.

Our team is working hard in different ways, including telephone and video consultations to keep the practice open and running effectively during this Pandemic. Face Coverings when attending the GP Surgery

If you are asked to come to the surgery for a face to face review/appointment, you will be required to wear a face covering to help keep you and us safe. You can use anything that covers your nose and mouth. Thank you.

We’ve recently launched a new system you can use to check your symptoms and submit forms to us.

Its available online by following the link below or via www.patientaccess.com. All you need to do is click the link and itll take you to an online form where you outline your symptoms. Youll then receive access to tailored and clinically approved articles and videos which will help you self-manage your condition.

The forms are sent directly and securely to us at the practice so we can check your answers and assess whether you need a face-to-face appointment with the GP or not and if not, well give you the advice you need on what to do next.

The forms are tailored to you and theyve all been approved by clinicians so the advice you get is really accurate and helpful.

You can fill in the forms at any time, day or night, so it means you have round the clock access to the information you need to manage your conditions.

We meet on the 3rd Tuesday of the month from 10:00 until 12:00.

Each month we have different speakers or activities.

Join us for a friendly chat and a cup of coffee / tea.

We offer support and advice for you as a carer.

All carers are very welcome to bring their cared for along to our groups.

What's on in March

Walking Group

We are trying to identify & support as many Carers as we can.

Particularly those people who may be looking after a member of their family or helping a friend or neighbour with day to day tasks, don’t really regard themselves as a Carer and are undertaking this vital activity without help or support.

If you are caring for someone, we really would like you to let us know, so that we can ensure you receive all the support and information we can give, on topics such as benefit entitlement, access to respite care or maybe simply being there to provide a kindly ear when things get too much.

If you are a Carer, please ask our Receptionist for a Carer’s Identification Form.

Carers Packs are also available in the noticeboard which is in the surgery entrance hall.

Please complete this form and hand it to our Receptionist who will give you a Carers pack.

Please download a copy of the North Staffs Carers Association Referral Form below. Please complete and email or post. The surgery will be happy to fax this for you as well.

If you would like to give permission for your carer to have access to your personal details and medical records held by the practice, please complete the Consent Form and bring it into the GP surgery so that your records can be updated
