New Patient Registration

To register with the practice you must live within the practice boundary. If you are new to the area we can register you immediately. If you wish to transfer from another doctor's surgery in the area we would need further information from you.

To register please visit reception and fill in a registration form. It would be helpful if you could bring with you your NHS card or number. You will be registered with the practice rather than an individual GP and may see any of the doctors. You can express a preference for which doctor you wish to register with and if possible we will register you with that doctor. You can express a preference of which practitioner you wish to see and if possible the practice will arrange this.

As part of the registration procedure you will be asked to attend an appointment with a Health Care Support Worker or Practice Nurse for a new patient check. This enables us to ensure we have the correct details and to be aware of any current medical problems or medications. You can only register with us if you agree to attend this appointment. Usually it should be within one month of wishing to register with the practice.

All staff within the National Health Service have the right to work without being subjected to violent, abusive or threatening behaviour. The practice will not tolerate violent or abusive or drunken behaviour towards any staff members or other patients. The practice reserves the right to remove these individuals from the practice list. If necessary we will also call for Police assistance.

Visiting the UK?  You may be chargeable for NHS treatment.

Whilst you can register with this GP practice as an NHS patient and see the GP without charge, you should be aware that not every person is entitled to all their NHS care free of charge in England.

Generally, NHS care is free for those:

ordinarily resident in the UK;

insured by an EEA member state, eg with a valid EHIC or S1 form;

covered under the Immigration Health Charge ("Surcharge");

exempt in law -eg refugees or victims of modern slavery (see over).

Interpretation Services

Interpretation services are available at the surgery.  The surgery can book a telephone interpreter with Language Line.  It also have the facility to translate a document or letter.

For patients who we consider require the services of an interpreter, please ask the reception team to put a Major Alert on the Patient Clinical Record, stating this fact and also detailing the language that they require to be spoken.

Surgery Location

From Macclesfield on the A523 enter Leek, continue straight through to Stockwell Street, turn right into Ball Haye Street then first left into Regent street.

From Buxton enter Leek on the A53, turn left into Ball Haye Street then first left into Regent Street.

From Cheddleton enter Leek on the A520, turn right into Brook Street A53, at the next roundabout take the second exit into Ball Haye Street then take third right into Regent Street.

From Ashbourne enter Leek on the A523, at the roundabout take the third exit into Ball Haye Street then take the third right into Regent Street.

Parking Facilities

There is a small car park to the rear of the building with one designated disabled parking area. There is a pay and display car park at the bottom of Regent Street and the top of Regent Street.

We request that patients do not leave their car in the car park whilst they go shopping so that other patients who visit the surgery may also use the car park.

Disability access to the building is via the back door and if anyone needs to use it, please ring the bell and someone will let them in. The front door has got a ramp, but a small step into the building.

Online 'Pre-Registration' With The Practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to provide (2 forms of id, a photograph and a name and address) and sign the form to confirm that the details are correct. You will not be registered at the practice until you have completed this registration process.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us either by  email, post, or by hand.

Note that by sending the form you will be transmitting information about yourself across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
