How to register with a GP Surgery
We are using a new online service called Register with a GP surgery that makes it easy to register with this GP surgery.
Just fill in this quick online form to start the process. Register with a GP surgery (gp-registration.nhs.uk)
You do not need proof of address or immigration status, ID or an NHS number if you already have an NHS Login.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Please call into the surgery or contact our reception team either by phoning them on 01538 399008 or emailing on moorland.med@nhs.net if you cannot download or access this paper form above.
As part of the registration procedure you will be asked to attend an appointment with a Health Care Support Worker or Practice Nurse for a new patient check. This enables us to ensure we have the correct details and to be aware of any current medical problems or medications. You can only register with us if you agree to attend this appointment. Usually it should be within one month of wishing to register with the practice.
All staff within the National Health Service have the right to work without being subjected to violent, abusive or threatening behaviour. The practice will not tolerate violent or abusive or drunken behaviour towards any staff members or other patients. The practice reserves the right to remove these individuals from the practice list. If necessary we will also call for Police assistance.
Interpretation Services
Interpretation services are available at the surgery. The surgery can book a telephone interpreter with Language Line. It also have the facility to translate a document or letter.
For patients who we consider require the services of an interpreter, please ask the reception team to put a Major Alert on the Patient Clinical Record, stating this fact and also detailing the language that they require to be spoken.
